A 5-star anomaly commission hidden inside a “normal” hospital visit. Four divergent endings, three projector slots, one invulnerable monster — and the most lore-heavy run NTE has shipped so far. Full clear in roughly two hours if you bring the right unit.

TL;DR — Optimal Run Order

  1. Talk to the patientHethereau Municipal Hospital, first-floor stairwell — the NPC facing the corner. Pick the “(Close Eyes)” line to enter the anomalous zone.
  2. Clear Ending 1 first“The Price of Betrayal” is the easiest route and rewards a Staff Elevator Card that drastically shortens the next three runs.
  3. Run E2 and E3Surveillance Records (E2) and Anomaly Experiments (E3) both share the upper-floor elevator unlocked from Ending 1.

Unlock & Access

The Nightmare-Bound commission does not appear on the Anomagram by default — you have to discover it by talking to a hidden NPC. Travel to Hethereau Municipal Hospital on Onion Road in Illusion Town. From the lobby, turn left toward the first-floor stairwell. The patient facing the corner is the trigger.

Interact and pick the dialogue option “(Close Eyes)”. The Nameless Hospital menu opens — confirm to enter the anomalous zone. From this point on every ending starts identically: clue collection diverges, but spawn, hospital layout and Bandaged Monster patrols are shared.

Once unlocked, you can re-enter the zone at any time from the same NPC — no need to re-clear the “discovery” step on subsequent endings.

Core Mechanics

Bandaged Monster

Invulnerable patrol on every active floor. Cannot be killed or stunned. Hide behind machinery, inside lockers or behind corners and break line-of-sight. Footsteps and audio cues are the safest tells — wait for them to fade before moving. Getting caught warps you back to the last save point but does not reset gathered clues.

1F Generator puzzle

Four idle generators must be powered in a fixed sequence: middle row first → its facing neighbour → the unit at the opposite end → finish on the one nearest the door. Activating them all spawns the Bandaged Monster: sprint to the exit on its lurch animation. Restores hospital lighting and the staff elevator.

2F Flooded Corridor (E2 and E4 only)

On a random check, the 2F outer hallway floods. Avoid the inner corridor. Look for the door with a collapsed wall on the right — inside, the water valve in the maintenance room drains the level. Skip this entirely on Endings 1 and 3.

Save points and run loops

Each floor has 1–2 wall switches that anchor a checkpoint. Always flip them before risky routes (Bandaged Monster patrols, jigsaw assemblies, timed file rooms). Death rolls you back to the last switch, not the whole hospital.

The Price of Betrayal

EASIEST

The required Anomaly Commission clear. Everything you need lives on 1F. Earn the Staff Elevator Card here — every other ending depends on it.

Walkthrough

  1. Spawn 1F. Head right to the monitor desk and inspect the active screen → Anomaly Disorder I.
  2. Loop the corridor: left through the doorway, right past the auto-closing door, right at the ER sign, hug the vending wall to the generator door.
  3. Run the generator sequence (middle → its neighbour → far end → near door). Sprint out when the Bandaged Monster spawns.
  4. Pick up the 1F Keychain on the desk behind the generators and flip the save switch.
  5. Through the door past the vending machines, into the first room on the right with the mannequin → inspect the desk monitor for Anomaly Disorder III.
  6. Cross to the operating room, exit left, head to the bloody-wall room. Door on the left of the splatter → desk monitor → Anomaly Disorder IV.
  7. Two rooms further on, pull the 1F Spare Key out of the lockers, destroy the trash bags blocking the door.
  8. Cross to the first double-doors, climb the stairwell to 2F. Skip floor 3.
  9. Enter the video room directly behind the 2F reception desk. Place the clues in order: Projector 1 → I, Projector 2 → III, Projector 3 → IV.

An Offering to Evil

MEDIUM

Same hospital, harder navigation. The 2F outer corridor may flood, forcing a detour through the maintenance room. Use the elevator unlocked from Ending 1.

Walkthrough

  1. Speak to the patient again, “(Close Eyes)” to re-enter. Take the elevator straight to 2F.
  2. Surveillance Record I: in the room across from the elevator, follow the zigzag path between the storage shelves to the dead end with the wooden mannequins → inspect.
  3. Take the elevator up. From the doors, turn right and follow the corridor straight until the blood-splattered wall, then left.
  4. If the corridor is flooded: follow the wall to the door with a broken right-side wall → operate the valve in the maintenance room to drain it.
  5. Continue to the second bloody wall (flickering light), turn left, enter the door marked by the red glowstick on the floor.
  6. Surveillance Record II: between the two desk monitors. Then flip the wall switch — a bulletin board reveals the 2F Keychain.
  7. Out, turn right, into the maintenance room (first door on the right). Surveillance Record III sits on the bulletin board left of the door, attached to a book.
  8. Loop back to the 2F reception area. Clear the three enemies guarding it.
  9. Video room. Projector 1 → I, Projector 2 → II, Projector 3 → III.

Medical Abomination

HARD

All clues live on 3F. Introduces the jigsaw assembly puzzle and two timed file rooms. The Bandaged Monster patrols this floor — plan stops behind doors.

Walkthrough

  1. Elevator straight to 3F. Inspect the sealed canister next to the blue fire extinguisher → North District Key.
  2. Through the double doors next to the canister, first door on the right, then right again into the room with the blue canisters.
  3. Investigate the notes on the central counter → Anomaly Experiment Clue I (informational — not used to trigger E3 but mandatory for Ending 4 prep). Grab the Jigsaw piece from the blue canister to its right.
  4. Cross to the room with the square central table. Pick “Assemble” — kill the minor spawns that drop in.
  5. Exit east. The Bandaged Monster spawns far down the corridor: backtrack into the Jigsaw room, wait for footsteps to fade, then sprint past.
  6. Run to the corridor where the Monster came from, take the first door on the right → desk → Anomaly Experiment II.
  7. Back to the Jigsaw room, exit south through the double doors. Past the save point, find the wall clock on the left and inspect it twice → South District Key.
  8. Open the double doors next to the clock. Reception desk on your right, enter the file room — first cabinet on the left of the shelves → Office Key. The file room has a timer; leave and re-enter if it expires.
  9. Out and run straight to the next door. Through, right, first door on the right → Anomaly Experiment III on the left desk. Move fast — this room also has a timer.
  10. Exit, first door on the left. Right from the desk, forward, second left past the save point, elevator down to 2F.
  11. Right out of the elevator, sharp left at reception, into the video room. Projector 1 → II, Projector 2 → III, Projector 3 → IV.

Voice of the Nightmare

HARDEST

The story-rich finale. Requires a character that can clear a ceiling hole — Esper Zero, Aurelia or Nanally. Three jigsaw pieces and a timed-switch bookshelf puzzle gate the final clue.

Walkthrough

  1. Elevator to 2F. Left, straight, then right at the windows with the bloody wall. Resolve the flood if it triggers (same valve as Ending 2).
  2. Enter the door with the red glowstick — flip the switch, take the 2F Keychain from the bulletin board.
  3. Exit, left, right at the corner. The Bandaged Monster patrols here: wait behind a corner, then trail it at a distance. Second door on the right → close it behind you. Jump the broken-wall gap into the room with the ceiling hole.
  4. Reach the ceiling. The exact tech depends on your unit: Male Zero double-jumps off the IV stand by the window and uses Esper Skill on the corner; Female Zero jumps onto the lockers, casts Esper Skill, swaps mid-air to a taller character and double-jumps through; Aurelia simply double-jumps off the IV stand and lifts; Nanally Esper-Skills onto the wall, then jumps repeatedly to flip through.
  5. Secret room above: inspect the blue canister left of the desk → first Jigsaw piece. The room safe holds 50,000 Fons — open it.
  6. Drop down, smash the trash bags to leave. Right at the corner, run past the reception enemies, elevator to 3F.
  7. Sealed canister near the blue fire extinguisher → North District Key. Into the double doors next to it, assemble the first Jigsaw on the central table — a hidden door opens west.
  8. Hidden room: right turn, run through the illusory wall at the far end. Inspect the desk notes repeatedly until V???'s Notes drops.
  9. Back to the table, exit east, first door on the right → second Jigsaw piece from the right-side sealed canister. Return, assemble, clear the minor spawns.
  10. Exit east, let the Bandaged Monster pass, right at the corner, first door on the right → Anomaly Experiment III on the desk.
  11. Cross to the room directly opposite → sealed canister on the right end gives you Mysty, an experimental object that follows you.
  12. Out, right into the corridor, first hallway on the left, first door on the right. Interact with the leftmost-table sealed canister → Mysty becomes the third Jigsaw piece.
  13. Loop back to the Jigsaw table, assemble. Out the east door, straight down the hallway (mind the Monster), right at the corner, second door on the left — the fallen-bookshelf room. The Monster will not enter.
  14. Bookshelf switch puzzle. Press the switch in this rhythm: tap twice fast → tap, 1s → tap, 1s → tap, 1s → tap, 1s → tap twice fast. A hidden door slides open.
  15. Inspect the computer on the long counter → Anomaly Experiment IV (the final clue).
  16. Right out of the bookshelf room, left back to the Jigsaw room, south through the double doors, right to the elevator → 2F.
  17. Right out of the elevator, left at reception, into the video room. Projector 1 → V???'s Notes, Projector 2 → Anomaly Experiment III, Projector 3 → Anomaly Experiment IV.

Pro Tips & Common Pitfalls

  • Run order: Ending 1 → Ending 2 → Ending 3 → Ending 4. E1 unlocks the elevator, and the 3F layout you learn during E3 directly maps to E4.
  • On every clue inspection, the screen sometimes prompts twice. The clue only counts on the second prompt — pull the desk a second time if your inventory looks empty.
  • The Bandaged Monster has scripted patrol arcs. If you wait inside a room with the door closed it never enters — use this on every uncomfortable corridor.
  • Endings 2 and 4 share the same flood mechanic and the same valve room. Pre-clearing the flood in E2 trains your route for E4.
  • Save the bookshelf switch puzzle (E4) for a calm moment — the 1-second timing window is unforgiving and the Monster can re-enter the area if you stall.
  • Two endings reward the optional Fons safe in E4's secret 2F room. Always open it on your first E4 attempt — easy +50,000 Fons.
